Guaranteed Issue Term Life Insurance.

Life insurance helps provide you and your loved ones financial security by protecting against the financial risk associated with death. The most common type of life insurance, term insurance, pays the benefit amount listed below. It covers you for a specific time period and has no cash accumulation feature. You cannot be turned down for this coverage. There are no health questions asked and no physicals to take.

Member Life Insurance Benefits

Accelerated Death Benefit for members who become terminally ill. Doctor certification required of terminal illness with condition expected to result in death within 12 months. Waiver of Premium is available for members who become totally disabled prior to age 60, and remain continuously disabled for 6 months. Conversion Privilege is available upon termination of employment. The member and/or dependent may be able to elect conversion to an individual policy within 31 days of termination of group coverage.

Dependent Life Insurance Benefits

Dependent life benefit is available for the member's spouse for the amount shown above. The dependent child benefit is $100 for age 8 days to 6 months. The child benefit is the amount shown above for children age 6 months to age 19, or to age 23 if the child is a full-time student.

Benefits Terminate

1. When active participation in the association ends
2. When the member ceases to be actively at work on a full-time basis
3. When the policy is discontinued
4. When the member (or dependent) reaches age 65
5. Retirement

Bereavement Counseling for Beneficiaries

In cooperation with Health Management Systems of America (HMSA), the insurance company is pleased to offer a toll free counseling service to all household members who experience the loss of a loved one. This service is available at no cost, 24 hours a day, seven days a week.

Effective Dates

Available effective dates are the 1st of each month. Fully completed applications must be received in The Select Company website by the 25th of the month in order to receive an effective date for the 1st of the following month. If received after the 25th, applicant must wait until the following month. For example: Application is received on July 24th; effective date will be August 1. Application is received on July 26; the effective date will be September 1.

What is the maximum age for children to be considered dependents on the Family Plan?

Children are considered dependents until their 19th birthday. Full-time students are considered dependents until 23rd birthday.

How do I change my beneficiary?

After you receive your policy synopsis, you simply call the company at the toll-free number provided and inform them what your desired beneficiary is.

What is the Insurance Carrier that covers the Life insurance portion of the membership package?

The blanket group policy is held by NAIB and is fully insured by an "A" rated insurance company. "A" rating provided by A.M. Best Company.

How do I add a new born to my family plan?

Once your baby is 8 days old, you simply call the toll free number provided in your policy synopsis to inform them of the addition to your family. You new born will be instantly covered as a dependent on your plan.
